Main Instrument The HMS survey questionnaire was designed specifically for this study. The instrument comprises a core of 138 brief items derived from validated, clinically relevant screening tools, such as the Patient Health Questionnaire (PHQ) measures of depression and anxiety.
In addition to these core measures, HMS allows participating institutions to integrate up to 10 additional questions of their choice if they purchase custom questions.
HMS uses skip patterns and logic to minimize respondent burden by eliminating questions that are made irrelevant by respondents’ answers to previous questions. As a result, the average respondent will take 15-20 minutes to complete the survey.
